STATUS: Full-Time; 40 hours/week
HOURS: Flexible, to meet the needs of consumers and their families
LOCATION: Throughout Allegheny County

 

Allegheny Children’s Initiative, a subsidiary of Partners For Quality Inc., is a non-profit organization that provides mental and behavioral health community-based services to children, adolescents and families throughout Allegheny and Washington Counties. Based in Pittsburgh’s South Side, we provide Intensive Behavioral Health Services, Service Coordination, Student Assistance, Parent-Child Interaction Therapy, and Family Based Mental Health programs. All employees of Allegheny Children’s Initiative are expected to act and conduct services in a socially just and culturally competent manner. Employees are supported in these efforts through ongoing development and training.

Responsibilities:

The  Community Health Worker will be responsible To meet the integrated physical/behavioral health needs of Allegheny County residents receiving services. The Community Health Worker (CHW) have a close understanding of the community that they serve and build a trusting relationship with community members, including those from diverse backgrounds, to connect those individuals to resources and care. The effectiveness of the CHW lies in their ability to patiently coach and support individuals that may live in underserved communities and assist them as they work towards health care goals by addressing the Social Determinants of Health (SDoH) needs.

 

 

Duties include, but are not limited to:

  • Demonstrates and reflects a commitment to the mission and values of Allegheny Children’s Initiative (ACI) by serving others with respect, dignity, and compassion.
  • Complete a standardized Social Determinants of Health (SDoH) assessment for each person receiving services, initially and quarterly, for the duration of service.
  • Once assessed, document measurable goals that address gaps in care.
  • Monitor, review, and update the goals with the individual on a consistent basis as goals are achieved or new SDoH gaps are identified.
  • Provide referrals and linkages to address the SDoH identified such as food security, housing, access to medical services, transportation, peer supports, education/career training, legal assistance, and social supports through health-related, home and/or community-based services.
  • Advocate for vulnerable populations within the health care system and the community at large.
  • Assist referred individuals with completing necessary forms to enroll or maintain HealthChoices coverage.
  • Assist in creating connections between vulnerable populations and the health care system by assisting with scheduled appointments as needed.
  • Assist individuals in obtaining preventative services by referring and linking the individual to a primary care physician.
  • Help individuals manage chronic conditions through education about their physical/behavioral health treatment plan and work with the provider to ensure the plan is acceptable to the individual.
  • Promote wellness by encouraging cultural competence and humility among health care professionals serving vulnerable populations.
  • Represents the agency to provides resources and connection at community outreach events.
  • Collaborate and provide support to the housing provider assigned to ACI.
  • Build relationships and interact professionally with local health and social service providers.
  • Completes all record-keeping and paperwork requirements promptly and accurately in accordance with agency policy.
  • Complete all releases of information forms to maintain compliance of required agency/DPQ standards.
  • Complete unusual incident reports/child line reports according to agency policy.
  • Other duties as assigned or requested.
  • Complies with Agency personnel policies and procedures and department policies and procedures.
  • Flexibility in schedule and availability to meet individual’s needs. This may include evening and weekend hours.

Qualifications:
  • High School Diploma or GED equivalent
  • One year of experience in a CASSP or mental health setting preferably with consumers under the age of 18
  • Associate’s degree with major course work in psychology, social work, or public health administration, preferred.
  • Act 33/34 clearances and FBI clearances required
  • Lived experience among the population to be served
  • Experience with or openness to working with diverse populations
  • Knowledge of chronic health conditions preferred
  • Complete Community Health Worker training and certification within 1 year of hire and maintain certification requirements yearly.
  • Sufficient computer skills to complete documentation in an electronic health record, assist individuals with internet-based benefits enrollments, and complete other assigned duties.
  • Valid Driver’s license and access to personal vehicle
Minimum Compensation: USD $42,000.00/Yr.
