Job Description

Summary

The Direct Care Professional (DCP) provides compassionate support and assistance to adults with disabilities across any setting, both onsite and in the community. The DCP ensures a safe, nurturing, and inclusive environment while assisting individuals in daily living activities, personal care, and social engagement.

Duties

• Provide direct care and support to adults with disabilities in accordance with their individualized care plans.
• Assist with daily living activities, including grooming, and toileting.
• Support individuals in meal preparation, feeding, and dietary needs as required.
• If authorized to do so, administer medications as directed and document accurately.
• Assist with mobility, transfers, and use of assistive devices as needed.
• Encourage and facilitate participation in social and recreational activities.
• Provide companionship and emotional support while promoting independence.
• Maintain a clean, safe, and organized environment.
• Monitor and report any changes in health, behavior, or needs to the appropriate supervisor.
• Adhere to all organizational policies, procedures, and DBHDS regulations.

Requirements

• High school diploma or equivalent required.
• Previous experience in direct care, home health, or disability support preferred.
• Certification in CPR and First Aid.
• Ability to lift and transfer individuals safely.
• Strong communication and interpersonal skills.
• Patience, empathy, and a commitment to enhancing the quality of life for individuals with disabilities.
• Work may be performed in a center based or community setting.
• Requires physical activity, including standing, lifting, and assisting with mobility.
• May involve exposure to varying medical conditions and behavioral challenges.
