The Pines Home Health and Hospice - Hospice Aide CNA / Certified Nursing Assistant

As a Hospice Aide CNA, you will play a crucial role in providing personal care and promoting comfort for our patients. You will work under the supervision of a registered nurse and be responsible for observing patients, reporting observations, and documenting care provided. This is a time-sensitive role that requires compassion, empathy, and a commitment to delivering high-quality care.

Job Summary:

• Perform personal care needs and attend to patient requests promptly
• Ensure patient safety and use equipment properly
• Provide personal care including baths, oral hygiene, and changing bed linen
• Assist patients with dressing and undressing
• Plan and prepare nutritious meals
• Assist with feeding patients
• Take and record vital signs
• Provide proper care and observation of patient's skin
• Report patient's condition and significant changes to the Case Manager
• Assist with ambulation and exercise
• Offer and assist with personal hygiene needs
• Perform simple procedures as ordered
• Keep patient's living area clean and orderly
• Adhere to documentation and care procedures
• Participate in quality assessment performance improvement activities
• Respect patient and family/caregiver environment and personal needs

Job Requirements:

• Successful completion of a state-approved training program
• Ability to read and follow written instructions
• Understanding of hospice philosophy
• Comfortable providing specialized care to the terminally ill
• Good oral and written communication skills
• Good organizational skills
