We are seeking qualified Independently Licensed and Intern Therapists to join a compassionate, supportive team of professionals in the Arizona area. You will have the freedom to design your own schedule, choose your desired caseload, and work in a setting that best suits you—onsite at Skilled Nursing Facilities (SNFs) or remotely via telehealth.
As part of our team, you'll have the opportunity to work with a group of mental health professionals dedicated to providing high-quality behavioral health services in Skilled Nursing Facilities (SNFs). We foster a collaborative environment with accessible leadership, ensuring you have the resources and support you need to excel in your role.
Responsibilities:
Conduct diagnostic evaluations and assessments, provide therapy, and develop treatment plans for individuals in Skilled Nursing Facilities and via remote telehealth.
Complete all necessary documentation in a timely manner using our user-friendly EMR system (Athena I).
Adhere to agency policies and procedures, ensuring quality care and compliance.
Job Types:
Full-time or Part-time opportunities.
Both W2 Employee and Independent Contractor options available.
Work Onsite or Remotely.
Requirements:
Master's degree in Social Work, Counseling, or a related field.
Arizona licensure as an LCSW, LMSW, LMFT, LAMFT, LPC, or LAC.
National Provider Identifier (NPI).
Professional Liability Insurance.
Credentialing with Medicaid/Medicare is a plus.
Some facilities may require updated vaccinations or appropriate exemptions.
Tuberculosis (TB) test clearance.
CPR, AED, and First Aid certification.
Desired Skills:
Strong computer skills and familiarity with basic software.
Excellent written communication abilities.
Attention to detail and organizational skills.
Ability to build rapport and communicate effectively with patients and team members.
Ability to remain flexible, organized, and work effectively under pressure.
Spanish language proficiency is a plus.

Competitive salary range: $50,000 - $96,000 (depending on licensure).
