Job Description

Rehabilitation & Long-Term Care Pharmacist
Support patient recovery and independence through personalized medication plans.

Key Responsibilities:
  • Review and monitor complex regimens in skilled nursing or rehab facilities.
  • Coordinate care with therapists, physicians, and caregivers.
  • Educate patients and families on medication goals and outcomes.

Qualifications:

  • PharmD with LTC, SNF, or rehab pharmacy experience preferred.
  • Compassionate communicator with strong organizational skills.
  • Familiarity with Medicare Part D regulations.
